Our statement vendor identified a statement error involving facility branding only—the correct patient received their own information, but it appeared on the wrong facility statement (Pawnee Valley Community Hospital – PVCH instead of Powell Valley Healthcare – PVHC).
No information was shared outside of the intended recipient, and this is not a HIPAA issue. Additional safeguards have been implemented.
